If you have fallen or are at risk of falling, our specialist service for adults can offer support and advice to reduce your risk of falling in the future.
 
COVID -19: The Falls service is accepting all referrals and the service is operating although we are currently unable to run therapy clinics and exercise classes are being offered via Zoom. All referrals and visits will be clinically prioritised.
 
The service is operating as one integrated Therapy Team with Physiotherapy and Occupational Therapy. This will be reflected on the SPE form as one service.

Overview

Therapy led service offering multi-factorial assessments for people that have fallen or are at high risk of falling.

We offer one-to-one assessments with a qualified therapist (Occupational Therapist or Physiotherapist) which take approximately 1.5 hours. The therapist will look at strength, balance and the potential causes of falls.

At the end of the assessment a care plan will be formulated depending upon your individual need. This may include:

    • Falls prevention education and advice
    • Tailored home exercise programme
    • Postural stability exercise groups (*assessment required with therapist)
    • Home hazard assessments
    • Further investigation of unexplained falls
    • Onward referrals or signposting to other services
    • Recommendations to community exercise groups

 

The current average waiting time for the Falls service is 5.8 weeks.

Who this service is for?

  • If you have fallen and you do not know why you are falling
  • If you fall frequently
  • If you’re worried about falling and this is stopping you carrying out your everyday activities

Using this service

Your GP or healthcare professional will be able to refer you to our service.

In order for us to manage your referral effectively, we require some information about how recently you have fallen and how often you have been falling. We would also like to know how worried you are about falling. The questionnaire below may help you to think about this.

We will sometimes re-direct you to a more appropriate team when we triage your referral. For example, to the Community Team; this is to ensure that you are being seen by the right person at the right time.

 

 

 

 

 

Appointments

Due to COVID-19 restrictions we are currently not able to provide face-to-face therapy clinic appointments. Telephone assessments are carried out initially, followed by a face-to-face home visit as needed. Consultant clinic initial assessments are currently being carried out over the telephone.

We offer therapy clinic appointments in locations across the borough (see the bottom of the page for clinic locations and times). For those that are housebound, we are able to arrange a home visit.

Additional ‘vestibular’ clinics are run on a required basis.

We will send you notification of your appointment in the post.

Cancelling an appointment: please give us as much notice as possible if you need to cancel your appointment, in order for us to be able to offer this appointment to somebody else.

 

Exercises

The videos in the link below have been created to help you with your exercises. You can watch them as many times as you like to help improve your technique. Your therapist will have selected exercises for you so please only do the exercises suggested for you.
