Falls and Fracture Prevention

If you have fallen or are at risk of falling, our specialist service for adults can offer support and advice to prevent you from falling in the future.

COVID -19: The Falls service is accepting all referrals and a full service is operating. All referrals and visits will be clinically prioritised. The service is operating as one integrated Therapy Team with Physiotherapy and Occupational Therapy. This will be reflected on the SPE form as one service.

Using this service

Your GP or healthcare professional will be able to refer you to our service using the Falls Referral Form.

In order for us to manage your referral effectively, we require some information about how recently you have fallen and how often you have been falling. We would also like to know how worried you are about falling. The questionnaire below may help you to think about this.

Patient Falls Questionnaire

We will sometimes re-direct you to a more appropriate team when we triage your referral. For example, to the Community Team; this is to ensure that you are being seen by the right person at the right time.
