The Care Coordination Centre is available on Weekdays between 8am – 5pm.

Patients calling regarding the District Nursing Service are able to contact us 24 hours a day, 7 days a week, by calling us on the above number, and selecting option 1, and then option 1.

If you are a patient and not certain which service you require, a call to the Care Coordination Centre is the quickest way to ensure you are directed to the right place.
