The rehabilitation service, either at Foxbury or in their own home, offers patients a period of rehabilitation to help them recover after a stay in hospital.

Overview

Bromley Rehabilitation service is comprised of two pathways; Bromley Bed-based Rehab and Home Pathway.

Bromley Bed-based Rehab is for patients who require additional rehabilitation support without the acute care provided in a hospital, but are not well enough to return home and Home Pathway offers additional rehabilitation support once a patient is discharged to their own home.

The service supports all patients living within the London Borough of Bromley who are registered with a Bromley GP and who are able to engage in regular therapy sessions whether that be within the Bed Based Unit or their own home.

The Bed-based Rehab service is a nurse led unit with the provision of a daily Visiting Medical Officer and weekly support from a Consultant Geriatrician.

Home Pathway offers rehabilitation alongside the usual medical cover of the patient’s own GP but also has weekly support from a Consultant Geriatrician.

Using this service

The Bromley Rehabilitation service uses a referral system; each referral is then triaged by a senior member of the team to ensure that the patient meets the criteria to access the service.

Referrals are made by the therapists within the hospital and multiple conversations are held to identify the safest discharge option for the patient.

On admission

Bromley Bed-based unit is a 24/7 service. Patients are greeted by a clinical member of the team on arrival and introduced to the Nurse looking after them for that shift.

Home Pathway covers the hours 08:00 until 20:00, 7 days a week and on admission offers a “Meet and Greet” from one of the Nursing Rehabilitation Assistants.  These are non-clinical members of staff who have been trained to ensure safety at home and introduce the service. The day after admission the patient will be assessed by a clinical member of the team to determine future needs and visits.

On discharge

Before being discharged from either rehabilitation pathway, future options will be discussed with patients and their families.  This includes but is not limited to, further rehabilitation (beds to home pathways if suitable), social services input and additional equipment that may be required in the home. All suggestions are made to ensure patient safety on discharge from the service and prevent further admissions to hospital.
