Overview
The service provides support between 8am and 8pm, seven days a week to those who become unwell and who without this service may call an ambulance and could make an unnecessary trip to hospital.
The service provides care in your own home with an Advanced Nurse Practitioner visiting you and to make an immediate assessment, offer support and guidance on the spot and arrange follow-up care if needed.
Using this service
To use this service you’ll need a referral from your doctor (GP) or another health care professional. You may also be referred to this service if you call 111 and they think you would benefit. The London Ambulance Service might also call the team if they think you would benefit from our care.
Your visit by the Rapid Response team
Your visit from the team will be in your own home. The person who visits you will assess you and discuss what next steps to take. The team may arrange follow up care if you need it and make a referral for you to see someone from another service. They will also let your GP know about any care you have received.
