Care Coordination Centre Launch

Bromley Healthcare is pleased to announce the launch of our Care Coordination Centre (CCC) on Monday 4 June 2018.

The CCC will give our patients, their families and health care professionals a single telephone number to call that will enable them to be routed through to the most appropriate person, department or member of staff. Our phone lines will be available between 8.00am and 10.00pm, 7 days a week so that patients can always contact us.

As well as being a single point of contact the CCC will process, clinically review and offer appointments for all of our services. The centre is based at Global House in Hayes and is a relatively new initiative for community health services nationally with very few providers offering such a co-ordinated single point of access to their patients.

People’s expectations on how they interact with healthcare providers is changing and in order for us to deliver effective services Bromley Healthcare has adapted to meet these demands.

Jacqui Scott Bromley Healthcare Chief Executive said “Our new patient centric Care Coordination Centre has a significant supporting role to play in ensuring continuity and consistency of patient care. It will proactively support the management of the 21,500 patients under our care, making it easier for patients to get the right information and care at the right time.

Option 2 – GPs and heath care professionals

For GPs referring into the service, this will still operate via the Single Point of Entry referral form. If you have any queries please use the existing service lead contacts or call into the new CCC number (option 2) if you do not have a direct contact.
