Overview
Our Twilight and Night Nursing Services provide essential care to patients in the community outside of standard working hours. We are here to support patients and their families when they need us most.
Service hours
Twilight Service: 6:30pm–10:30pm
Night Service: 10.00pm–7:00am
What we do
• Provide essential nursing care at home during evenings and nights.
• Help prevent unnecessary hospital admissions through timely interventions.
• Support patients with ongoing treatment needs outside of daytime hours.
• Work closely with GPs, District Nursing, hospitals, and specialist teams to ensure continuity of care.
Using the service
For patients already known to the service, you can refer yourself.
You can also be referred by:
• GPs
• District Nursing teams
• Hospitals and LAS
• Specialist services
